AT TIME OF SERVICE
Coinsurance, copay fees, and deductible fees are determined by your insurance company. These fees must be paid by you to our office at time of service. You are required to bring your physical insurance card to your first appointment.

SELF PAY
There are advantages to self-pay over using insurance. When you self-pay, your confidentiality remains exclusively between you and our office. This helps maintain your privacy.

When you use insurance we might be required by law to disclose your personal information to your insurance company to process your claim.

You can choose to self pay if we are not in-network with your insurance.
