We also make every effort to avoid a long wait to get you in for your first visit.

Please register on the same day as our initial phone conversation so we can hold your first scheduled appointment for you.

Registering ahead of time provides substantially more time for counseling during your first visit.

HOW TO REGISTER AFTER OUR FIRST CONVERSATION

Fill in all boxes on page 1 and page 2 of your registration.
Make sure you enter all numbers accurately in to the boxes.
Make sure each form has your DocuSign entered and saved.
Click the big green SAVE & CONTINUE TAB on both pages.
