Instructions for 1st page:

Fill in all boxes & enter all #s accurately - do not leave any boxes blank.
Upload front & back of your insurance & license cards on to the registration.
Bring in your plastic insurance cards to your first session.
Click same as client only if you are listed as primary insured on your policy.
Enter the PRIMARY insured’s policy # and subscriber #.
Enter secondary insurance info only if you carry a supplemental policy.
Additional info entry: enter ALL the requested information.
Enter Partner info if your partner may participate in the therapy.
DOB entry: only use the mini calendar.
Click the GREEN ”SAVE & CONTINUE” tab which takes you to the 2nd page.

Instructions for 2nd page:

You will now see typical consent forms.
Proceed to Agree, Consent and Save each separate form.
DocuSign your full signature on each separate document.
Click the GREEN ”SAVE & CONTINUE” tab to submit your registration.

Note: Once you register online, we immediately receive it. Your online registration is the prerequisite for keeping your 1st session for the time slot that we scheduled for you.

We set aside significantly more time for you at your first session. This allows us to become better acquainted with you and your situation.

This extra time at your first intake session is free of any extra charge.

If you need to reschedule your first session, please leave a voicemail to let us know at least 48 hours ahead.
